Error lodging visa

Guys sino po naka encounter na while lodging visa nag eerror po siya doon sa dependents details. Ganito po ang nalabas after clicking next right after migrating companion on page 3 “ an error has occured, the related application details provided cannot be verified, check.the details and amend if necessary. Ilang beses ko po chineck yung details ko and paulit ulit po na ganyan yung response. Dinilete at inulit ko din po ang pag pi fill up ng info pero ganun pa din. I hope someone could help me kasi nasakit na yung ulo. Hindi ko po alam saan ako nagkamali
