Dependents under 18 requirements

hello po and happy new year sa lahat..

crineate ko po ang new discussion na ito kasi need ko po ng help/clarification/insights about sa requirements ng dependent under 18, baka meron same experience/situation sakin at mashare kung anu yun prinepare nila..
single mom kasi ako pero yun anak ko is naka family name sa father niya… walang problema sa father willing xa mag sign any consent form .. ito kasi yun Nakita ko na requirements para sa visa 491(photo attached)
pwede lang either sa statutory declaration and form 1229? or dapat both? naghahanda lang kasi ako kasi mag oopen na ang NSW sa february and nandon ang skillcode ko.. nasa UAE kasi ako now and since uuwi ako ng Cebu for 30days vacation so might as well iprepare ko nalang din ang document na ito para incase if mainvite ako and nandito ulit ako sa UAE is ready na lahat

sa form 1229 din , ang last page meron details ng 2 parents,, so meaning pipirma din xa? ang father ng daughter ko? and also does he need to provide all passport biopage, driver's license and national id and birth certificate, or dapat kahit 1 lang sa mentioned? attaching photos sa form 1229. thank you in advance and sorry sa mahabang query.. and happy new year ulit sa lahat. sana magandang balita ang awaiting satin lahat ngayon 2021.
