@kjjarina Yep nurse po ako. So pwede po sa kahit saang clinic yung immunizations or titer test? Not necessarily sa panel clinic? Kasi nung nag inquire ako sa panel clinic about immunizations hinanapan ako nag form from the school ung list ng requir…

Hi just wanna ask where did you get the list of requiremnts of immunization po? Does the school provide the form? I dont have my childhood immunization records kasi so i want to have the titer test and all asap coz it might take longer im living out…
